Please complete the following form to register for the Semaglutide program at Rejuvené Med Spa and Wellness Center. Upon successful submission of the registration form below, next steps include:

Clinical Intake Forms - You will immediately be redirected to the required intake forms. You will also receive an email with a link to the required intake forms. All forms must be completed before labs are ordered.
Lab Work - Once all intake forms are received, your lab tests will be ordered for the location and date you request below.
BOOK YOUR APPOINTMENT! - Once lab work has been received by our office, you will receive a link via email to book your initial appointment with Dr. Reed.

You must complete all intake forms online and your lab tests in order to book your first appointment to begin injections.
